Just be gentle with degree of forward pressure on device and reduce tension as it starts to advance. Stop inflating if you see valve expanding. This is what happens if you don’t…

@evandrofilhobr @RodrigoBagur @terumoisuk Dedicated sheathless gear can be limited by availability of your favourite shapes. Terumo have 7F slender sheaths but often use standard guides in a sheathless manner. Easiest is 🐈‍⬛ with 5 or 6F diagnostic in 7 or 8F guide. Tend to use pigtails or MPA - 125cm lengths

@AhmadAlkhalilMD @nolanjimradial Failure of device with intra-or extra-vascular deployment is rare. May be managed with manual pressure or balloon tamponade. Need for vascular surgery is very rare. We switched overnight to Manta. Have used it for our last 500+ cases.

@MohanedEgred @SVRaoMD Cordis Adroit 125cm 6F guides. I use MPA1 for radial bail out of large bore femoral problems, but they have other shapes if you need.

@ZeroTLR Interesting that an intra-annular device gives similar haemodynamics to supra-annular device? Maybe it’s just that self expandable valves have better haemodynamics compared to balloon expandable?

@drandrewsharp Wanted a cutting balloon, but had got rid of them all, in favour of more deliverable scoring balloons. Though if I had used one, I might not have been as confident of getting through. The visual confirmation with septals filling with rupture was very reassuring at the time!
