As many doctors in the UK, I have been worried about the current state of training, the use of MAPs in medical rotas, unemployed GPs and resident doctors. I had a look at the competition ratios (CR) in Specialty training and though I'd show you some of the data 🧵 1/n

Normal lymph node secondary follicle and IHC (immunohistochemistry). Not exhaustive but enough to start! What is IHC you ask? It’s something pathologists use to stain different cells and helps us tell between different diseases. #PathTwitter #hemepath #MedTwitter

Let's start with GP, the number of applications has increased year on year, on 2023 there were 10,514 applications for 3, 935 posts. See how the number of posts is relatively flat at around 4,000 jobs per year?. Below is the CR, last year there were 2.67 applicants per job 5/n

@medicalmodelbri I worked as a senior clinical fellow in urology for years, I then decided to switch to Histopathology. I am happy I had to start from zero or I would be a danger to my patients. This idea of being a generalist after two years is dangerous and shows a frightening lack of insight

Anaesthesia has a similar problem (Here CT1). More and more people applying, and a completely flat number of posts available . But the number of posts has actually dropped! There were 629 in 2018, and only 545 in 2023. I thought we had a shortage of doctors? 9/n

This is not exhaustive, but I have included some graphs showing the number of applications vs number of posts, and at the bottom the competition ratio (CR), that is how many people apply for one post. The higher the CR, the more difficult it is to get a training post. 2/n

Paediatrics was a bit difficult, here is CT1. Same trend, more applications as time goes on, number of posts remains fairly flat and the CR increases year on year. I don't know if I am correct but from the data it looks like these Drs have to reapply every year to progress 6/n

Neurosurgery ST2/3, I had to mix this one as well as this changed, in 2020 it was an ST3 application and in 2021 changed to ST2. What happens to the ST1's/ST2's that don't make it? Is the CR getting lower because people are leaving? 16/n

@smartymartycavy @TomStocks1982 I don't understand the argument that they have a degree in biomedical sciences. I have worked with great scientists, that does not mean 2 more years makes them qualified to see patients. It is not about time spent in Uni, it is about WHAT you are learning.

@Ask_foradoctor Perhaps better wording would be completed x number of eviscerations. PMs are always done by Pathologists. APTs are vital as they are skilled at evisceration, and depending on the mortuary 4 cases in the morning is not unusual. The examination of the organs is always done by a Dr

@OpenAcademics @MonicaMalta7 It is possible to have a family and have a career. I started my PhD when my son was 1.5 years old and my sister has 3 children and just passed her defense. You’ll learn to be a bit more strict with time and maybe a bit more juggling, but those are skills you learnt on your PhD.

I also created a pathological pTNM, where you may notice there is no pT1, and pT2 is organ confined disease (no a, b and c). The pathological TNM is obtained after the prostate has been resected (taken out), that's why you cannot have pT1! #Uropath @urorao
